• An original Power of Attorney signed by trademark owner;
• Photocopy of original Certificate of Trademark Registration.

II. TRADEMARK AMENDMENT

1. Required documents

• Original notarized Power of Attorney;
• Original notarized Declaration of Change of Name/Address or legal document showing the change of name/address;
• Original Certificate of Trademark Registration.

2. Time limit
